• Signup to ref a game in Stack Officials You must be an official Referee Volunteer in AYSO 190 to signup to ref a game.
• AYSO Incident Report Form - This form is used whenever there is a personal injury, damaged property or threats of or actual physical violence surrounding an AYSO game, practice, event or property. The form should be completed by the coach, AYSO Official or other AYSO Volunteer. Return the completed form to [email protected].
• Referee Game Report (referee use only) - After each game, the center referee is required to fill out this brief online game report. This helps keep the referee staff informed of how games are going. The feedback provided will also help us ensure a positive experience for all members of the “Kids Zone” – Players, Coaches, Referees and Parents.
  
• Soccer Accident Insurance Claim Form - AYSO purchases Soccer Accident Insurance ("SAI"), which pays excess medical costs up to $50,000 maximum per accident to an insured person for accidental bodily injuries incurred as a direct result of participation in a covered activity subject to the policy terms, conditions and exclusions.  For a copy of the form email [email protected] to request one.
